To ensure a smooth and seamless transition, please review and complete the following steps. Steps for a Smooth Transition
Please call your insurance company to confirm that your current home address and phone numbers are up to date. You will also need to provide the following information:
Provider NPI: 1891003208 (Dr. Nichelle Adegbite-Maraventano, MD)
Please make sure these updates are effective before your first appointment.
Please upload a copy of your child's insurance card to the patient portal (if applicable).
Upload your child's vaccination records to the patient portal prior to your visit. This allows the provider to review them in advance.
As a courtesy, please inform your child's previous pediatric office that you wish to transition to another office. They should expect a medical records request from our office following your first visit.
Once you receive your updated insurance card reflecting our practice, please upload it to the patient portal. You may then proceed to schedule your child's appointment with Dr. Nichelle Adegbite-Maraventano, MD.
